New Patient Medical History Forms
This is an announcement to let dentists and our clients know that we can also create a custom new patient questionnaire to compliment their website.
These dental health and medical history forms are used to collect information from new patients. They can be downloaded from the dentist’s website, then filled in at home prior to the patient’s appointment.
Online dental forms are a convenient accessory that save the patient precious time spent during the office visit. Data entry solicited in the registration such as insurance, medical consent for exams, release of information, etc. can be edited to customize it for the individual practice. Colors and name of the dentist, or office displayed on the form are included at no extra charge.
